Instructions for ordering Custom Pet Memorial (slant)

1. Enter a 20 Character Sentiment (i.e. Companion & Friend)
2. Enter your pets name. (20 Character Limit)
3. Enter the dates you want to appear on the Memorial.
4. Choose Dog or Cat
5. Choose Graphic of Dog or Cat Breed
6. Choose Paw Print (if you want the paw print.)
7. Enter Billing and Shipping Information.
 

Cost is $95.00 including text and graphics
Flat Rate shipping and handling is $35.00. 

 
   
Choose Dog Breed View Pictures To Submit Your Own Dog or Cat Photo, please Email your photo separately from this form being certain that we have your complete name and billing address to locate the order. Artwork fee is $35.00 per hour minimum.  Please submit only 'head shots' saved as .psd (Photoshop) or Vector files if possible.  We will send you a separate invoice via PayPal for the custom artwork once you approve the charge.

After we receive your order, we will create your proof and email it to you for approval.  Your order must be paid before we will create a proof.  Once we receive your proof approval, we will send our order to production.  Production time depends on date of approval and is usually 2-3 weeks.

You will be directed to pay for your purchase on the Order Confirmation Page.  By clicking 'Buy Now' you will be taken to the PayPal site where you can securely pay with your credit card.

When your Pet Memorial is ready, we will send you an email with the tracking number.
